Chapters in the Family Medicine Midwest region are encouraged to send a minimum of two students from your state to attend.

• Chapter will receive scholarship donor special recognition on the FMM Foundation web site if they agree to ongoing annual support of at least two students from their state.

David Deci, MD Scholarship Award

David Deci, MD was one of the three founding members of Family Medicine Midwest.  He served as Chair of the FMM board in 2014 and remained on the board until he retired from the University of Wisconsin in 2020.  He remained connected to FMM as an emeritus member of the board until 2022.   Dr. Deci passed away in 2023 and this award was created in his honor. More about Dr. Deci can be found here.

2024 marks the first year of the David Deci, MD Scholarship Award.  The Scholarship opens June 1st and the application closes at Midnight on September 2nd.  The awardee of the scholarship will receive $1,000 which will cover their conference registration fee and travel expenses.  Dr. Deci was passionate about Family Medicine, Rural Medicine and Street Medicine to help those experiencing homelessness.

  • Applicants must have a poster to present at the conference on October 18, 2024
  • Applicants should have a passion for Family Medicine, Rural Medicine, and Street Medicine to help those experiencing homelessness.
  • All applicants will be reviewed by the Deci Scholarship Task Force and will choose the winner.
