Drawing for a Cruise Certificate

Healthcare Associates Credit Union (HACU) has donated a cruise certificate to the Family Medicine Midwest Foundation and all FMM 2019 attendees are automatically entered for a drawing for this certificate for an upcoming aquatic adventure!   You can increase your odds of YOUR NAME being the lucky one drawn by donating towards student scholarships before or on #GivingTuesday - December 3. 
If you weren't at FMM 2019, but have ever been involved with any of our conferences, you're now also eligible to enter just by donating!

Make your donation online to the FMM Foundation Scholarship Fund.  A $50 donation will provide five additional entries for the drawing.  Only online donations are eligible and must be made before midnight Central Time on December 3 to qualify you for entries into the drawing. 

The winner will be contacted by email and phone on December 4.  If the winner chooses to accept the certificate, they must activate the certificate within 45 days and pay all required port fees (approximately $424).  Cruise must be booked and sailed within 18 months.
