AAFP and AMA collaborate on Health Plan Complaint Form.  AMA will share quarterly reports with AAFP of family physicians who file complaints about private payer issues.  If you have a situation, please use this form to aid AAFP in gathering important data.

There are four ways to access and submit the forms.  After connecting to the AAFP Web site link located below, physicians or physician staff members can submit a complaint using one of the following options:

• Use an AMA Members Only password to login and submit a complaint online.
• Use an ME number and date of birth to login and submit a complaint online.
• Use an AMA Internet ID to login and submit a complaint online.
• Print the form and fax or mail it back to the AMA

All complaints are anonymous and confidential.
