It’s definitely a new era in state government with the beginning of the 96th Illinois General Assembly. There may never be a better time to get involved and get to know your elected officials! 

IAFP offers five different dates to participate in the legislative process in March or April. These “mini” lobby days are designed to give family physicians more opportunities to learn about our state government and advocate on issues that impact family medicine. You will meet face to face with those policy makers and legislators involved in creating and implementing health care policy in Illinois that affects you and your patients.

Each Spring into Action session will use the same template so that ANY date you pick will enable you to:
-Advocate on behalf of family medicine and its strengths
-Increase your visibility with legislators and policymakers 
-Exchange information about public policy and its effects on your practice and patients

**SB 1415 – FAMILY CARE EXPANSION TO 185% OF FPL
The Illinois General Assembly has passed SB1415. The bill solidifies FamilyCare eligibility at 185% of the Federal Poverty Level (FPL). It clarifies the law, which had been misconstrued by the courts, and ends the threat going forward that the program could be rolled back to 133% of the poverty level. 

**IMPEACHMENT TRIAL SCHEDULED TO BEGIN ON MONDAY
The impeachment trial of Gov. Rod R. Blagojevich is scheduled to begin in the Illinois Senate on Monday, January 26. The governor held a press conference on Friday afternoon where he declared the impeachment trial rules unconstitutional and that he would not participate in the process.

** ECONOMIC STIMULUS PACKAGE AND HEALTH CARE
Excerpted from AAFP News Now, by James Arvantes

House Democrats unveiled a draft version of the economic stimulus bill on Jan. 15. The proposal includes many priorities of the AAFP and can be a needed boost to primary care.

Important items in the stimulus package include: 

$95.6 billion to increase the federal government's Medicaid contributions to states, allowing states to maintain their Medicaid and SCHIP coverage programs, which are stretched to the limit by low-income individuals who are seeking coverage for their children and families. Illinois currently has one of the nation’s lowest FMAP (Federal Medical Assistance Percentage) rates. 

$20 billion to help physicians adopt health information technology, or HIT.

$600 million to train primary care health professionals, such as primary care physicians, dentists and nurses, while also helping medical students who agree to practice in underserved areas after graduation pay their medical school expenses.

$3 billion for a prevention and wellness fund to better manage and treat chronic and infectious diseases. The prevention and wellness fund would include support for hospital infection prevention programs, immunization programs and evidence-based disease prevention programs.

$1.1 billion for health care research and quality programs to compare the effectiveness of different medical treatments funded by Medicare, Medicaid and the State Children's Health Insurance program, or SCHIP. 

$1.5 billion in funding for community centers, including $500 million to increase the number of uninsured Americans who receive health care through the centers and $1 billion to renovate clinics and to upgrade HIT systems in the centers. 

$550 million to the Indian Health Service to modernize older hospitals and health clinics and to upgrade health care technology to improve care for underserved rural populations.

Other provisions would help people who lose their jobs keep their employer-provided health insurance longer than the current law allows.

UPDATE: AAFP has delivered a letter to the three House committees that will consider the Stimulus package outlining their support for many of the proposals and asking questions and clarifications on specific items. **ICD-10 IMPLEMENTATION DEADLINE EXTENDED TO 2013
HHS announced that it would extend the implementation deadline for the International Classification of Diseases, 10th Revision, Clinical Modification, or ICD-10-CM, for outpatient diagnosis coding, to Oct. 1, 2013.

The new deadline gives physicians, health insurers, software vendors and other stakeholders two additional years to prepare for a change that will increase the number of outpatient diagnosis codes from 13,500 to 68,000.

** US HOUSE PASSES INCREASE IN FEDERAL TOBACCO TAX 
The US House of Representatives passed two key bills on Jan. 14th. First they voted overwhelmingly to reauthorize the State Children’s Health Insurance Program (SCHIP). A separate measure passed by the House would provide a boost in funding. That bill increases the federal tax on cigarettes by 61-cents to $1.00 per pack. The new revenue would go to SCHIP. The bill is expected to be considered in the Senate later this month, where it is expected to pass. 

**FAM MED PAC REMINDER
By law, AAFP dues cannot be used for any candidate or election purposes. The only way for AAFP to contribute to elected officials is though the PAC. Consider just $1 per day, a $365 annual contribution (called Club George, as in Washington). If every member contributed to FamMedPAC, we’d have one of the strongest PACs in the nation. Any member of IAFP is allowed to contribute, that includes residents, students and chapter staff!

** JANUARY 27 DEADLINE TO APPEAL UHC RATING
Physicians should appeal their rating in United Healthcare’s (UHC) Premium Designation Program (PDP) by Jan. 27 if they believe it is wrong. According to a letter UHC sent Dec. 23 to its more than 220,000 participating physicians, only appeals received by Jan. 27 are guaranteed to be adjudicated and updated for the scheduled March release of new ratings for physicians in the PDP. Although physicians may appeal their designation status to UHC at any time, successful appeals received after that date may not be reflected in the initial release of new ratings.

To help physicians understand this, the AMA is sharing this question-and-answer e-mail that clarifies important aspects of UHC’s PDP and the appeals process.

How does the PDP work?
In UHC’s PDP, physicians who pass UHC’s quality check are awarded one star in the UHC directory. A second star is added to those who also pass UHC’s “efficiency,” or lowest cost of care, criteria. Participating physicians who do not pass the quality check are given no stars, and physicians with insufficient data to be rated are so designated. Some two-star physicians may also be eligible for a fee schedule update through the UHC Practice Rewards Program.

How accurate are the ratings?
The PDP is based on UHC claims data as measured against their physician performance measures for quality. Efficiency is measured using UHC’s episode treatment groups, or ETGs, methodology, which is solely based on cost-of-care factors. Like most physician profiling systems, UHC claims that its data is risk adjusted; however, the risk adjustment factors are limited, and the state of the art for this type of risk adjustment is still somewhat primitive. Questionable physician attribution techniques and small sample sizes used to determine ratings (as few as 10 patients’ records) also contribute greatly to rating results that are frequently inconsistent with the actual cost and quality of care being delivered.

What can I do about my rating?
Because of the unreliable nature of these results, physicians in the PDP, particularly those who were given only one or no stars for their rating, are encouraged to check the data and analyses that were used to create their ratings. By checking the information that was used to create the rating, physicians may find incorrectly attributed patient episodes or treatments that were due to patient preference or other extenuating circumstances. Physicians who find such errors can appeal the rating results and have them corrected before the ratings are ever used or published.

How do I see my data and appeal my results?
Physicians must visit https://ereports.uhc.com/ReportCard  and log in using their ID and password, which appears on the letter they received from UHC announcing their rating. Requests for reconsideration must be submitted in writing or electronically. Please include the Designation Detail report, with your comments, and the signed attestation.

Physicians who do not have their copy of that letter, experience difficulty with the Web site or do not understand their data are asked to contact UHC at (866) 270-5588. This number should be used only by physicians regarding their rating in the PDP.

The PDP has not rolled out in all areas of the United States, nor does it cover all medical specialties. Only physicians in areas where the program is effective and in specialties that are part of the program would have received the designation letter. ILLINOIS is one of the states where the PDP program is in use. 

Remember, appeals must be sent via e-mail, fax or postal mail and must be received by UHC by Jan. 27 to assure they will be reviewed prior to the publication of the physician ratings.

*Ben Brewer, MD of Forrest is back to work in 2009 with his twice-monthly column in the Wall-Street Journal called “From the Doctor’s Office.” 
The first of his January columns offered his arguments for the value of the annual physical exam and explained the benefits of the relationships built between family physician and patient that make these visits worthwhile, and potentially lifesaving. His January 21 column offers advice for reforming payment systems for Medicare, Medicaid and private payers to pay primary care for the many ways they provide care, keep patients healthy and keep costs down in the primary care setting.

Members can download and personalize any of the letters available on the AAFP's Private Sector Advocacy "Get Involved" Web page. The template letters are only accessible to members, so have your member identification number handy.
