Update: Illinois Health Connect (IHC)

For Providers  

  • IHC has enrolled 3,400 PCPs representing a capacity to care for 3.9 million patients throughout the state.
  • Providers can access a list of their patients or “panel rosters” through the IHC website at www.illinoishealthconnect.com.  Note: In order to see the rosters, providers might have to readjust the security settings and pop-up blockers on their computers.

The panel roster contains only the patients who have actively chosen that provider as a PCP.

Providers may have many patients who are “pending” auto-assignment (i.e. will be assigned to that PCP if they do not make an active choice.)

AHS is making the electronic panel rosters available in a CSV (similar to Excel) format so that groups can combine them or manipulate the data.  This option will be available soon.

A paper copy of panel rosters was sent to all PCPs in mid-March.  Due to the delay in client enrollment in Cook County , no panel rosters were sent in April but PCPs will receive one for May.

  • PCPs who registered to accept only their existing patients might have some patients who report difficulty enrolling with that PCP.

The ability to “link” a patient to a provider (and thereby define “existing patient”) is based on paid claims data.  There are several reasons why the AHS system might not be able to “link” a patient to a PCP - such as the patient hasn’t been seen in the past 3 years, the claim was rejected or the patient was seen by another provider in the group.

PCPs who identify patients who report being unable to enroll with them but wish to include the patient in their panels, can complete a form and indicate that patient should be enrolled as a “new patient”.  The form can be obtained from the Provider Helpdesk at 1-877-912-1999.

  • The current patient limit is1,800 per PCP and 900 for a resident, NP or PA.  PCPs can request a greater number of patients by contacting AHS who will forward a request to HFS.  Note: In order to preserve continuity of care, PCPs who have claims data documenting that they are currently caring for more than 1,800 IHC-eligible clients might receive an increase to the limit.

Regarding Client Enrollment  

  • Client enrollment started in the Collar Counties on Feb 16th and will be completed by the end of May.  At that time, all IHC-eligible HFS clients (Medicaid, All Kids, FamilyCare) will have a PCP.
  • Client enrollment started on Feb 16th in Cook County .  In order to accommodate the significant call volume and allow clients time to make an active choice for a PCP, the 60-day enrollment window has been extended indefinitely.
  • Clients who do not make an active choice for a PCP will be auto-assigned via an algorithm that looks primarily at paid claims data.  Due to some refinements, the auto-assignment process is being re-run for Cook County prior to auto-assigning any patients.  This will not affect any patients who have made an active choice of PCP.
  • Many Spanish-speaking families, who received their initial enrollment materials in English, were identified and received subsequent information in Spanish.
  • Examples of all the client enrollment materials in both English and Spanish are available on the Illinois Health Connect website at www.illinoishealthconnect.com.
