is the Family Physician of the Year?

Provides
patients with compassionate and comprehensive care.

directly involved in community affairs and activities to enhance the
quality of the community.

a credible role model professionally and personally to other health
professionals.

effectively represent IAFP and the specialty of family practice in
public speaking.

Is
in good standing in his or her medical community.

Exemplifies the tradition of the family
doctor and the contribution of family physicians to the continuing
health of the citizens of
Illinois
.
Eligibility
Requirements
Each candidate must:
1.
Be in active practice.
2.
Spend at least 50 percent of his/her time in direct patient
care.
3.
Be an IAFP member in good standing.
4.
Maintain a full-time practice.
Limitations
Members
of the IAFP Board of Directors and the Awards and Promotions
Committee, as well as previous recipients of the award are not
eligible.
Nomination and
Selection Process
Any
member of the IAFP or the public can submit nominations.
A nomination is simply a letter explaining why their nominee
should be Family Physician of the Year. Nominations are due by
December 31 and must include the nominees name, practice address and
phone number, as well as contact information for the person sending
the nomination. Click

IAFPís
Awards and Promotions Committee will review all nominees and select
final candidates to present to the Board of Directors for final
selection in March 2005 at the IAFP Spring meeting.
Candidates will be notified of the results by March 31.
The winner will be honored during the summer of 2005.
The winner will also become IAFPís
nominee for AAFPís Family Physician of
the Year Award in 2006.
The
2004 Family Physician of the Year is Steven M. Armbrust, M.D. of
Wheaton
.
