WANTED: FAMILY PHYSICIAN OF THE YEAR

There are over 2,100 active IAFP members out there – doing extraordinary things every day. One patient, one day at a time, family physicians are the detective, the coach, the counselor and the confidant, keeping Illinois families healthy. You know them, and we need to know them. Even if your favorite doctor has been nominated before – send her name in again! You can mail, e-mail, fax, use a carrier pigeon, anything to get the story of Illinois’ best family physician to our offices! The deadline for nominations is March 31, 2007. Use the form on the back of this sheet!

JUDGING CRITERIA

The Family Physician of the Year Award recognizes a physician who

  • Provides patients with compassionate, comprehensive and caring family medicine.

  • Is directly involved in community affairs and activities to enhance the quality of the community

  • Provides a credible role model professionally and personally to other health professionals, and residents and medical students

  • Can effectively represent IAFP and the specialty of family medicine in the public arena

  • Exemplifies the tradition of the family doctor and the contribution of family physicians in a leadership role improving the health of the citizens of Illinois.

Eligibility Requirements

Each candidate must:

1. Be in active practice.
2. Spend at least 50 percent of his/her time in direct patient care.
3. Be an IAFP member in good standing.
4. Be board certified in family medicine.
5. Be in practice at least five years since completing residency training

Limitations

Members of the IAFP Board of Directors, members of the IAFP Awards Committee, and previous recipients of the award are not eligible.

Nomination and Selection Process

Any member of the IAFP or the public can submit nominations. A nomination is simply a letter explaining why their nominee should be Family Physician of the Year.

IAFP staff will notify each doctor of his/her nomination. If nominee accepts, he/she or a designee will need to submit a CV, photo, and letters of support.

IAFP's Awards Committee shall review all nominees and select the top candidate to present to the Board of Directors for final approval as the Illinois Family Physician of the Year. Final selection will take place in June.
